Ken Kaneki first appears in the manga Tokyo Ghoul, created by Sui Ishida. He is initially a human college student with an avid love for books and a shy personality.

Kaneki's life drastically changes after a date with Rize Kamishiro, who is revealed to be a powerful ghoul. During the date, Rize attacks Kaneki, but she is killed in an accident. To save Kaneki's life, he undergoes an emergency surgery where Rize's organs are transplanted into him, transforming him into a one-eyed ghoul.

This transformation forces Kaneki to adapt to the ghoul world, struggling with his newfound hunger for human flesh and the moral implications of his existence. He becomes involved with the ghouls of Anteiku, a coffee shop that acts as a haven for peaceful ghouls, and learns to control his ghoul powers while trying to protect the people he cares about, both human and ghoul.
